Foundational Principles and Clinical Considerations for Traditional Lingual Orthodontic Appliance Set-Up Procedures

Lingual orthodontics has become an important esthetic alternative to conventional labial appliances; however, successful treatment outcomes depend largely on precise appliance positioning and accurate set-up procedures. Because access to the lingual surface is limited and the lingual morphology of teeth is highly variable, lingual orthodontics requires greater planning accuracy than labial treatment. This review summarizes the evolution of traditional lingual appliance systems and focuses on the fundamental principles of traditional lingual set-up procedures. Emphasis is placed on the role of the set-up model as the central reference for treatment planning, indirect bonding, and biomechanical control. Key considerations include the establishment of appropriate occlusal references, coordination of maxillary and mandibular arch widths, transverse and sagittal relationships, anterior tooth inclination relative to the occlusal plane, and the rationale for controlled overcorrection to compensate for appliance-related errors. Understanding these foundational principles is essential for translating treatment objectives into predictable and stable clinical results when using traditional lingual orthodontic appliances.
